ESCMID backs MMR vaccination in a CMI position statement, “Science, not politics”, endorsed by IDSA; it lands as US measles cases pass 4,500 and the UK SMI whooping cough consultation closes today.
ESCMID’s Executive Committee and Vaccines Subcommittee have published a statement in Clinical Microbiology and Infection (with a companion piece in CMI Communications), “Science, not politics: the ESCMID view on protecting populations against measles, mumps and rubella” (Brugger S, Fitzpatrick F, Friedland JS, Kluytmans J and colleagues). ESCMID flagged it on X at 07:45 GMT today, saying it is endorsed by SAS, EVASG and IDSA. The article summary says vaccine schedules and delivery “should be guided by robust scientific evidence, public health principles and cost-effectiveness, rather than political considerations”.
It comes as CIDRAP reports 4,588 US measles cases this year, more than twice last year’s total (23 Oct, covered yesterday). UKHSA’s measles data are still at the 15 Oct update, and the next release is due 12 Nov. In the UK, today is the last day to comment on UK SMI B 6 (Investigation of whooping cough). It was otherwise a quiet window: UKHSA, MHRA, ECDC and WHO DON published nothing new.
Check before quoting: the CMI and CMI Communications pages blocked our automated check (HTTP 403), so we’ve read only the title, author list (Crossref) and the summary sentence shown on the linked post, not the full text. We couldn’t confirm the online publication date. The US figure is from CIDRAP’s post, not CDC.
Counted from today’s digest. The Crossref sweep (online 25–26 Oct) found one infection item (OFID babesiosis); the CMI statement was found through ESCMID’s post. NEJM’s three new items were oncology. Links are unique content links across the issue, not counting our two website links. Gaps are the items in the Coverage gaps list; reports that simply haven’t been published yet are listed separately.

The other headlines from today’s issue. Each card links to its source and to the full summary. Agencies were quiet overnight, so most of today’s news comes from official accounts on X.
B 6 whooping cough consultation closes today
RCPath lists UK SMI B 6 (Investigation of whooping cough) as open 12–26 Oct; feedback is through the survey form linked on the page.
Nurse killed responding to an Ebola alert in Tshopo
Dr Tedros says a nurse was killed and another health worker severely injured in an attack in Bafwasende on 24 Oct.
Kenya: 4,900+ health workers trained; 42-day clock for Ebola-free status
The Health CS says there are no travel, trade or tourism restrictions. No new case count was published in the window.
Tshwane unchlorinated water outbreak: 595 cases reported to Parliament
Update to our 21 Oct item. Norovirus, Campylobacter and Shigella were found; no cholera. Case counts differ between sources.
Wisconsin: H5N1 in a commercial flock of about 13,000 birds
Barron County, reported on FluTrackers from a state release. Secondary; not checked against USDA.
IP-10/CXCL10 tracks parasitaemia and severity in babesiosis
A pilot cytokine study of 16 hospitalised patients and 7 controls (online 26 Oct).
What we could not confirm
The CMI statement’s full text and publication date (the publisher blocked our check). The DRC government’s reported 60 new Ebola cases and 46 deaths come from FluTrackers, not WHO. The Tshwane case totals differ by source (408, 433 and 595), and the plant manager’s suspension comes only from an X news summary. The Wisconsin H5N1 report is secondary. CDC HAN is still unreachable, and ECDC’s week 43 CDTR still isn’t on the archive.

UK SMI B 6: Investigation of whooping cough
The consultation runs from 12 to 26 Oct. Comments go through the survey form linked on the RCPath page. This was first forwarded by Mail Sorter on 20 Oct.
Measles and flu/COVID data unchanged
Measles is still at the 15 Oct update (next due 12 Nov). The latest flu/COVID-19 report (22 Oct) gave flu positivity at 6.3% and hospital COVID-19 PCR positivity at 8.9%. The next report is due Thursday 29 Oct. No infection mail arrived in the window.
ECDC / Europe
Nothing new from ECDC; Shamonda virus encephalitis paper circulating
ECDC’s newest items are still the Kenya/Nigeria Ebola threat brief (23 Oct) and the first human Shamonda virus infection in France (22 Oct). The week 43 CDTR still isn’t on the archive; the latest there is week 42. On 25 Oct FluTrackers highlighted the Eurosurveillance report of fatal encephalitis due to Shamonda virus Europe 1 (SHAV-EU1) in a severely immunocompromised patient in France, the same case ECDC announced on 22 Oct.
Global
DRC: health worker killed in Tshopo; Kenya sets out its 42-day countdown
- DRC, security: Dr Tedros (25 Oct) says a nurse was killed and another health worker severely injured on 24 Oct, attacked while responding to an Ebola alert in Bafwasende, Tshopo province.
- DRC, care capacity: WHO AFRO says the Isiro (Haut-Uélé) Ebola treatment centre has been expanded, and WHO has delivered more than a tonne of supplies, including oxygen equipment and PPE, to the general referral hospital.
- DRC, figures (secondary): FluTrackers relays DRC government figures of 60 new cases and 46 new deaths (31 community deaths), with a CFR of 48.6%. The reporting period isn’t stated and we haven’t checked these against WHO.
- Kenya: the Health CS says more than 4,900 health workers have been trained. Ebola-free status will be judged after 42 days in line with WHO, and there are no travel, trade or tourism restrictions. No new case count was published in the window (last: nine confirmed, one cluster).
- Vaccines: Africa CDC describes TUMAINI as an adaptive phase 2/3 trial that starts with Ervebo and then moves to Bundibugyo-specific candidates. It runs alongside frontline worker vaccination and the BRAVO healthcare worker study.
There’s been no new WHO DON since DON622 (23 Oct). Nigeria contact numbers and a weekly stipend for contacts appear only in an X-generated news summary, so we haven’t used them here.

Bronkhorstspruit, Tshwane: unchlorinated water linked to mixed-pathogen gastroenteritis
- Cause: the Bronkhorstspruit Water Treatment Works ran without chlorination from about 24 Sep to 14 Oct, drawing from a sewage-polluted river. DWS found very high E. coli and coliform counts in treated water and reservoirs (14 Oct).
- Microbiology (NICD): norovirus, Campylobacter and Shigella were detected in three of four stool specimens, together with other enteric pathogens. No cholera was found. Most cases are in children under four, and no deaths have been reported.
- Counts: 408 cases between 7 and 18 Oct (joint ministerial statement, 19 Oct); 595 between 7 and 19 Oct in health department figures presented to Parliament on 23 Oct (as reported by The Citizen), 262 of them in children aged up to four.
- In the window: an X news summary reports that the mayor has suspended the plant manager and ordered an independent probe. We haven’t confirmed this from an official source.
Numbers don’t line up: the official statement gives the peak as 72 cases on 13 Oct, but the presentation reported by The Citizen gives 75 on 11 Oct. Parliament also cited a media figure of 433. Moving from 408 to 595 means 187 cases added in a single day (47 reported for 19 Oct), so the later figure probably includes retrospective line-list additions. This is our inference. Tap water remains for toilet flushing only.

Wisconsin: H5N1 in a commercial flock in Barron County
FluTrackers (25 Oct) relays a Wisconsin government report of H5N1 in a commercial poultry flock in Barron, with about 13,000 birds affected.
Secondary; we haven’t checked it against the Wisconsin release or USDA APHIS.
Travel notices unchanged; HAN still unreachable
The travel notice list is identical to yesterday’s, including the Kenya Level 1 Ebola notice and the DRC Level 4/3 notices. The HAN archive returned an error page again. EID’s newest early-release items are still dated 19–23 Oct.
Journals
New since the 25 Oct issue (online 25–26 Oct), from a Crossref sweep of the 19 watched titles, plus the CMI statement found through ESCMID’s post. NEJM’s three new items were oncology trials, and the other titles had nothing new.

Science, not politics: the ESCMID view on protecting populations against measles, mumps and rubella
Brugger S, Fitzpatrick F, Friedland JS, Kluytmans J, Şahin GÖ, Paño-Pardo JR and colleagues (ESCMID Executive Committee and Vaccines Subcommittee; endorsed by SAS, EVASG and IDSA, according to ESCMID). The statement reaffirms support for MMR vaccination and argues that schedules and delivery should follow evidence, public health principles and cost-effectiveness rather than politics. A companion version is in CMI Communications.

Circulating IP-10/CXCL10 correlates with parasitaemia and disease severity in babesiosis: a pilot study
Plasma cytokines in 16 hospitalised patients with babesiosis versus 7 controls. Eight cytokines were raised (including IL-6, IL-10, IP-10 and TNF-α). IP-10 correlated with parasitaemia (Spearman R = 0.52, P = .04), and IP-10 and IL-5 were higher in severe disease. Immunosuppression was associated with low VEGF and basic FGF.
Takeaway: this is hypothesis-generating only. The sample is small, and IP-10 isn’t a validated severity marker.

What we could not confirm
We want you to know exactly where today’s issue has limits, so here are the 7 gaps we couldn’t close, plus the reports that simply weren’t out yet.
| Source | Issue |
|---|---|
| CMI statement | The CMI and CMI Communications pages returned HTTP 403 to our automated check, so we had only the title, authors (Crossref) and the summary on the linked post. We couldn’t confirm the online publication date or check the full text. |
| DRC Ebola | The 60 new cases, 46 deaths and 48.6% CFR come from FluTrackers relaying DRC government figures. The period isn’t stated and we haven’t checked them against WHO. |
| Tshwane | Case totals differ (408 official to 18 Oct; 433 media; 595 presented to Parliament, as reported), and so do the peak dates (13 Oct vs 11 Oct). The plant manager’s suspension is from an X news summary only. |
| US | The Wisconsin H5N1 flock report is secondary. The US measles total (4,588) is from CIDRAP’s post (23 Oct), not CDC. The HAN archive returned an error page. |
| ECDC | The week 43 CDTR still isn’t on the archive (latest week 42). We didn’t open the Eurosurveillance Shamonda article itself. |
| Nigeria | No official update in the window. The stipend and contact numbers are from an X-generated summary only. |
| Journals | Crossref sweep only, and indexing can lag; Elsevier titles (CMI, JHI, J Infect, IJID, AJIC) often index late. |
