Microregistrar Infection Newsletter · Saturday 24 October 2026
Kenya reports a second Ebola death as WHO publishes its first multi-country Bundibugyo DON and ECDC keeps the EU/EEA risk very low; UKHSA updates its flu antiviral guidance for critically ill patients.
On the evening of 23 Oct Kenya’s Ministry of Health reported the death of a 65-year-old woman, Kenya’s third confirmed case and a close relative of the index case. She developed symptoms in quarantine on 13 Oct. This brings Kenya to 2 deaths among 9 cases; seven patients remain in two isolation facilities and all cases are in one cluster with no evidence of community spread. WHO’s Disease Outbreak News (23 Oct, 16:03 BST) is the first to cover DRC, Kenya and Nigeria together: DRC 9,704 confirmed cases and 4,687 deaths (CFR 48.3%, to 20 Oct), 66 health zones in eight provinces, and the TUMAINI Phase 2/3 Bundibugyo vaccine trial launched in Bunia on 23 Oct.
ECDC’s threat assessment brief (23 Oct) rates the likelihood of infection for EU/EEA residents living in or travelling to affected areas of Kenya and Nigeria as low, onward spread after an import as very low, and the overall EU/EEA risk as very low. In the UK, the UKHSA influenza antiviral guidance was updated (14:32 BST) with new evidence on treating critically ill patients with oseltamivir and revised renal dosing, and the Ebola Returning Workers Scheme now covers Kenya and Nigeria.
Check before quoting: the WHO DON’s Kenya figures are to 20 Oct (one death) and predate the second death. Press reports differ on the date of Kenya’s first death (5 or 6 Oct). In Nigeria, Switzerland’s foreign ministry describes the patient as a Swiss woman, while Africa CDC called her Ugandan-born; both say she didn’t travel to DRC.
Counted from today’s digest. The eleven journal items come from CID (one), OFID (three), JAC (two), JAC-AMR (two), ARIC (two) and JID (one). Links are unique content links across the issue, not counting our two website links. The Kenya figure is from the Ministry of Health statement on 23 Oct. Gaps are the items in the Coverage gaps list; reports that simply haven’t been published yet (it’s a Saturday) are listed separately.
Also today
The other headlines from today’s issue. Each one links through to the full summary. It’s a weekend issue: UKHSA, MHRA and ECDC published nothing after Friday evening, and there was no new MHRA infection alert.
Second death: a 65-year-old relative of the index case
2 deaths among 9 cases; 7 in care; one cluster, no community spread (MoH, 23 Oct).
First multi-country Bundibugyo DON
DRC 9,704 / 4,687 (CFR 48.3%); 482 new deaths since 8 Oct, 307 of them in the community; TUMAINI vaccine trial starts in Bunia.
Kenya/Nigeria imports: EU/EEA risk very low
Low likelihood of infection for travellers who take precautions; no travel restrictions recommended; preparedness checklist for hospitals and labs.
Antiviral guidance: critically ill and renal dosing
Oseltamivir remains first line; REMAP-CAP preliminary data show no benefit of monotherapy in established critical illness started at a median of day 5.
Returning Workers Scheme adds Kenya and Nigeria
Applies to health and humanitarian workers deployed to the Ebola response in settings with possible direct exposure.
Cefepime–zidebactam phase 2: 91.7% clinical cure
72 treated; 82.4% of isolates MBL producers (mostly NDM); 86.7% cure on external adjudication; single-arm.
What we could not confirm
Kenya’s first-death date and the Nigeria patient’s nationality differ between sources, and her contact count is 44 or 46. One contact-tracing figure in the WHO DON doesn’t add up. We didn’t diff the flu antiviral guidance line by line. The CDC HAN, measles and Ebola pages returned 403, and ECDC’s week 43 CDTR wasn’t on the archive. Three journal items are title-only.
Microregistrar Moodle update
Two new entries. We read the Microregistrar moodlecloud sheet (Index tab, “Whats new?”) directly at about 07:57 BST. The 23 Oct issue reported the 22 Oct conjunctivitis item; both entries below are dated 23 Oct.
Interactive HYN – Anthrax
New interactive high-yield notes on anthrax. This is the sheet entry only; we didn’t open the Moodle page.
Interactive HYN – PVL Staph aureus

Agencies & guidance
The main developments today are Kenya’s second Ebola death, WHO’s multi-country DON, ECDC’s threat assessment brief, and UKHSA’s updated flu antiviral guidance going into winter. There’s no new MHRA infection alert, no new HCID list change and no CDC travel notice change in the window.
Guidance changes
Influenza: treatment and prophylaxis using antiviral agents updated
The change note says the update “incorporates new evidence on the treatment of critically ill patients with oseltamivir and updated recommendations on renal dosing”. Critical influenza is defined as needing ICU-level organ support (NIV, invasive ventilation and/or vasopressors). Oseltamivir remains first line, with the greatest benefit within 48 hours; starting up to 5 days after onset is off-label but may help. A new note says preliminary REMAP-CAP data in critically ill patients over 12 years show a lack of benefit from oseltamivir monotherapy in established critical illness, where it was started a median of 5 days after onset. Where oseltamivir can’t be started promptly or given, consider baloxavir (alone or with oseltamivir), or zanamivir INH/IV if baloxavir is unsuitable, under specialist advice.
- Renal: don’t reduce oseltamivir treatment doses in mild-to-moderate impairment (eGFR 31–60); keep 75 mg twice daily. If eGFR is the only value available, don’t delay treatment. Use CrCl, not eGFR, for IV zanamivir.
Action: update local flu treatment pathways and ICU and renal dosing tables before the season gets going.
We read the critically ill and renal sections but didn’t compare them line by line with the previous version. Check the tables before you change local doses.
UKHSA / MHRA / England
Ebola and Marburg Returning Workers Scheme: Kenya and Nigeria added
Current outbreak areas are now DRC, Kenya and Nigeria. For Kenya and Nigeria the scheme covers health and humanitarian workers deployed to the Ebola response in settings with possible direct exposure. For DRC it covers all workers returning from affected areas, plus all health and humanitarian workers returning from anywhere in DRC. Specific high-risk areas in CAR, South Sudan and Uganda are still listed.
Action: ask about RWS registration and deployment role when assessing returning responders. The HCID list is unchanged since the 22 Oct update.
ObLong: asymptomatic COVID-19 testing in acute NHS hospitals
A new observational longitudinal study of how routine asymptomatic testing of healthcare workers in 2022 affected transmission, and its cost-effectiveness. It also looks at barriers trusts faced in implementing testing policy. We read the summary only and didn’t extract any findings.
Measles unchanged; no new MHRA infection alert
Measles data are still at the 15 Oct update; the next scheduled update is 12 Nov. MHRA’s only items in the window were two non-infection approvals (alopecia areata and psoriasis). Mail: no Mail Sorter forwards and no UKHSA, RCPath or Green Book alerts arrived in the microregistrar2021 mailbox in the window.
ECDC
Ebola imports to Kenya and Nigeria: overall EU/EEA risk remains very low
- For people from the EU/EEA living in or travelling to affected areas of Kenya and Nigeria, the likelihood of infection is low if they follow recommended precautions. Further spread after an imported case in Europe is very unlikely.
- ECDC asks countries to keep preparing for imported cases: raise awareness among clinicians, and make sure hospitals and laboratories are ready to identify and safely manage suspected cases. A readiness checklist for national authorities is provided. No travel restrictions are recommended.
- The Nigeria case flew in from Uganda on 3 Oct, with onset on 7 Oct, admission on 11 Oct and confirmation on 20 Oct.
The week 43 CDTR wasn’t on the ECDC archive at compile time; the latest there is week 42 (16 Oct). There was no new Eurosurveillance content in the window.
Global — Ebola (Bundibugyo)
Kenya: second death among nine cases
- MoH (official, on X): the ministry “mourns the death of a 65-year-old woman who was receiving care for Bundibugyo Ebola”. “Seven patients continue to receive care. All nine cases remain within one cluster, with no evidence of community spread.”
- Press (BBC, Daily Nation, The Standard, AP, Reuters): she was the third confirmed case and a close family member and direct contact of the index case. She developed symptoms in quarantine on 13 Oct and was admitted the same day. The seven still in care are three relatives of the index case, three health workers and a hospital cleaner (BBC). Kenya has traced 135 contacts, all quarantined (Reuters).
Doesn’t add up: the BBC and Reuters give 5 Oct for the first death, the Daily Nation 6 Oct. Reuters says four countries are affected; WHO counts five (DRC, Uganda, France, Kenya, Nigeria).

Nigeria: patient “showing signs of recovery”; Swiss ministry says she didn’t visit DRC
- Federal Ministry of Health (via Channels TV, TheCable): the patient is showing signs of recovery and there’s no evidence of community spread. Contact tracing, quarantine and monitoring continue.
- Swiss foreign ministry (via Channels TV/AFP): she is the wife of a Swiss member of staff at the Swiss embassy in Abuja. Infection may have happened on a trip to Uganda about two weeks earlier, and neither she nor her husband went to DRC. He is in isolation without symptoms, and repatriation to Switzerland is being considered.
- Africa CDC (on X, 24 Oct): there is “no validated information yet” that she travelled to DRC. Africa CDC is convening African health ministers on 28 Oct.
- WHO DON: 44 contacts identified and monitored as of 20 Oct. Vanguard reports 46 contacts on an aviation no-fly list (44 hospital contacts plus 2 domestic staff).
Check before quoting: this is press-reported. The Swiss statement calls her a “Swiss woman”, while Africa CDC said earlier that she was Ugandan-born; the reports aren’t necessarily incompatible but haven’t been reconciled. We found no NCDC situation report.

WHO
Ebola disease caused by Bundibugyo virus: DRC, Kenya and Nigeria
- DRC (to 20 Oct): 9,704 confirmed cases, 4,687 deaths and 2,595 recoveries (CFR 48.3%), across 66 health zones in eight provinces. 976 cases and 482 deaths since the 8 Oct DON; 307 of those deaths were in the community and 175 in treatment centres. 67 new cases in the latest 24 h. Ituri 7,040 cases / 3,271 deaths; Nord-Kivu 2,146 / 1,199 (CFR 55.9%). Dakwa (Haut-Uélé) is the most recently affected zone.
- Contact follow-up was 79.7% (24,017 of 30,130), below the 95% target; Nord-Kivu was 73.3%.
- Vaccine: the TUMAINI Phase 2/3 trial of Bundibugyo vaccine candidates enrolled its first participants in Bunia on 23 Oct.
- Risk: still as assessed on 14 Aug (very high for DRC, high for neighbouring countries, low for the rest of the region and globally); the rapid risk assessment is being revised. WHO advises against travel or trade restrictions.
Doesn’t add up: Tshopo is given as 83.7% (1,077/12,086), but 1,077/12,086 is 8.9%. The provincial denominators as printed sum to 40,930, not 30,130; a Tshopo denominator of about 1,287 would fit both figures. The Kenya data stop at 20 Oct (one death).
Takeaway: 63% of recent deaths occurred in the community, so late presentation is still the problem. Keep a high index of suspicion for febrile travellers from DRC, Uganda, Kenya (Nairobi) and Nigeria (Abuja).
CDC / US
DRC Level 4 and Level 3 notices unchanged
Level 4 (Ituri, Nord-Kivu) and Level 3 (Haut-Uélé, Tshopo) are still dated 4 Aug. Yellow fever notices for Colombia and Ghana are unchanged, and we saw no Kenya or Nigeria notice. There was no MMWR in the window (next Thursday).
HAN, measles and Ebola situation pages returned 403
Our requests were blocked, so we couldn’t check for new HAN notices, the measles count or the CDC Ebola summary.
Scrub typhus mimicking typhoid in MDS (China), and zoonotic Pteropine orthoreovirus in Indonesian fruit bats
Both were posted on 23 Oct at 05:00 BST, just before the window opened, and weren’t in yesterday’s issue. Titles only.
Takeaway: scrub typhus can present as an enteric-fever-like illness in immunocompromised hosts.
Journals
New since the 23 Oct issue (online 23–24 Oct), from a Crossref sweep of the 19 watched titles. There were no new infection items in Lancet Infect Dis, Lancet Microbe, NEJM, IJID, ICHE, AJIC, J Hosp Infect, J Infect, CMI, AAC, JCM or Eurosurveillance. JAMA had only a news brief on WHO reporting the highest number of cholera deaths in decades (not listed). The OFID acalabrutinib aspergillosis report was covered yesterday.
Clinical Infectious Diseases
Cefepime–zidebactam for carbapenem-resistant Gram-negative infections, including MBL producers: phase 2, open-label, single-arm
72 adults treated for 7–21 days. Of 68 qualifying isolates, 56 (82.4%) were MBL producers, mostly NDM. Investigator-assessed clinical cure at test of cure (CME, n=60) was 91.7% (95% CI 81.6–97.2%), and 93.9% in MBL infections. External adjudication gave 86.7%. 28-day mortality was 2.8%, and treatment-related adverse events occurred in 15.3%.
Takeaway: promising activity against NDM, but there was no comparator, and the adjudicated cure rate was about five points lower than investigators’. It isn’t a replacement for aztreonam–avibactam outside trials yet.
Open Forum Infectious Diseases
Pilot study of oral linezolid for early syphilis
15 of 18 patients on linezolid had an adequate serological response at six months, against 5 of 6 given benzathine penicillin G. There were no serious adverse events.
A small pilot; the comparator arm had six patients.
Penicillin allergy delabelling in nursing homes: barriers and facilitators
A qualitative study of 25 participants in five Massachusetts homes. Labels persist unquestioned, and structural barriers are decisive. History-based assessment was seen as the most feasible approach. HCPs cited cognitive impairment as a barrier, yet 13 of 21 residents screened could give meaningful histories.
Self-collected dried blood spots for HCV, HBV and SARS-CoV-2
110 of 388 recruited completed the study. HBsAg 100% sensitivity and specificity; HCV antibody 96.2% sensitivity (25/26) and 96.4% specificity (81/84).
Only 28% completed. The acceptability figure (101/129) uses a different denominator from the 110 completers, and the abstract doesn’t explain why.
Journal of Antimicrobial Chemotherapy
Probenecid as a pharmacokinetic enhancer of meropenem in sickle cell disease

No clinically relevant QTc prolongation with ceftibuten or ledaborbactam etzadroxil
Concentration-QTc analyses from two phase I studies (84 healthy adults) of the oral combination in development for cUTI caused by MDR Enterobacterales. Neither agent showed a clinically relevant effect on ΔΔQTcF.
JAC-Antimicrobial Resistance
Carbapenemase-producing Enterobacterales in Peshawar, Pakistan
Of 32 isolates, 90.6% carried blaNDM-5, often with OXA-48-like genes (46.9%). ftsI insertions and CMY co-production were linked to higher aztreonam–avibactam MICs in E. coli. Reduced cefiderocol susceptibility was seen mainly in NDM E. coli ST2 with cirA disruption.
Takeaway: relevant to UK patients with healthcare exposure in Pakistan. Test aztreonam–avibactam and cefiderocol susceptibility rather than assuming it.
ESBL-E transmission from animals, environment and hospital surfaces to humans in Singapore: expert elicitation

What we could not confirm
We want you to know exactly where today’s issue has limits, so here are the 8 gaps we couldn’t close, plus the reports that simply weren’t out yet. It’s a Saturday, so most agencies won’t publish until Monday.
| Source | Issue |
|---|---|
| Kenya | The press gives 5 Oct (BBC, Reuters) or 6 Oct (Daily Nation) for the first death. Reuters says four affected countries; WHO says five. The second death is from the MoH statement and press, and isn’t yet in any WHO or ECDC figure. |
| Nigeria | The patient’s nationality (Swiss per Bern, Ugandan-born per Africa CDC earlier) and the contact count (44 per WHO/FMoH, 46 per Vanguard) haven’t been reconciled. We found no NCDC situation report. Reports are press-based. |
| WHO DON622 | The Tshopo contact follow-up figure (1,077/12,086 = 8.9%, not 83.7%) doesn’t add up, and the provincial denominators don’t sum to 30,130. Kenya data stop at 20 Oct. |
| UKHSA flu antivirals | We read the critically ill and renal sections but didn’t compare the guidance line by line with the previous version, so other changes may have been missed. The ObLong findings weren’t extracted. |
| ECDC | The week 43 CDTR wasn’t on the archive at compile time. The threat assessment brief was read in summary only. |
| CDC | The HAN archive, measles page and Ebola situation summary all returned 403. |
| Journals | Crossref sweep only, and indexing can lag. The JAC probenecid paper and both ARIC papers are title-only. The OFID DBS abstract has inconsistent denominators (flagged). |
| Mail / X | No Mail Sorter forwards or infection alerts arrived in the window. The X check was light, and the x-api-mcp-guide skill wasn’t available to load. The Nigerian “15 states” and C. auris stories weren’t verified. |
