Microregistrar Infection Newsletter · Tuesday 20 October 2026
ECDC updates its Ebola page with all 8 Kenya cases: EU/EEA importation risk still very low, but higher for returning healthcare workers.
ECDC’s Ebola page, updated 19 Oct 14:35, now covers all 8 Kenya cases, including 1 death, and DRC data to 17 Oct (9,455 / 4,561). It still rates the risk of importation into the EU/EEA as very low, with the caveat that returning healthcare workers have substantially higher exposure risk. In the UK, Kenya stays on UKHSA’s HCID country-specific risk list (entry last updated 15 Oct): use it for VHF risk assessment in returning travellers, including healthcare and hospital support staff. The CDC has also issued a new Level 1 travel notice for Kenya.
No new UK statement: UKHSA hasn’t published anything new on Kenya, and the UK surveillance figures are unchanged. Kenya’s case details still come from health ministry briefings reported in the media, and the latest DRC figures come from a SitRep summary site.
Counted from today’s digest. The eight journal items are from Lancet Infect Dis, ICHE, OFID, AJIC, CID (two, including the Fleming Fund supplement) and IJID (two); the JAMA guideline synopsis is under Guidance and the J Infect review under Watched pages. Links are unique content links across the issue, not counting our two website links. Gaps are the items in the Coverage gaps list; reports that simply haven’t been published yet are listed separately.
Also today
The other headlines from today’s issue. Each one links through to the full summary. UK flu/COVID-19, measles and Salmonella figures are unchanged; week 43 flu/COVID is due on 22 Oct at 14:00.
DRC: 9,552 cases, 4,602 deaths
+137 cases and +66 deaths since SitRep 155 (CFR 48.2%). 97 new cases on 18 Oct; 30 of 41 deaths in the community; Nord-Kivu treatment centres over capacity (450 patients, 410 beds).
Taken from the ebola-tracker.org SitRep summaries, not WHO or ECDC.
Kenya: still 8 cases, 1 death
All seven earlier patients have received monoclonal antibodies and are responding well, according to the Director-General. No approved vaccine or specific treatment for Bundibugyo virus.
Press-reported.
Level 1 Ebola notice for Kenya
Usual precautions; avoid contact with symptomatic people and body fluids. No new HAN, and MMWR is still the 15 Oct issue.
Oropouche in pregnancy
About 3-fold fetal loss (aRR 3.08) and haemorrhage (aRR 2.79); CNS abnormalities in 8% of infants of infected mothers vs none.
IV-to-oral switch under-used in BSI
Only 32.3% of eligible patients were switched by day 7; switching was linked to shorter stays.
MRSA and PVL S. aureus updated
Two refreshed teaching entries on 19 Oct.
What we could not confirm
Kenya’s figures come from health ministry briefings via the media, with no official ministry sitrep and no WHO Disease Outbreak News yet. The DRC figures come from a SitRep summary site. Several CDC pages blocked us, and some journal items were title-only.
Microregistrar Moodle update
Two new entries since the last check (newest first).
MRSA (updated)
Refreshed teaching item on meticillin-resistant Staphylococcus aureus.
PVL Staph aureus (updated)
Refreshed item on Panton–Valentine leukocidin-producing S. aureus.

Agencies & guidance
ECDC’s Ebola update, DRC SitReps 156 and 157 and a new CDC travel notice for Kenya are today’s main developments. UK surveillance and WHO are unchanged since yesterday.
Guidance changes
Complicated UTI in adults (IDSA 2025 guideline)
A JAMA Clinical Guidelines Synopsis (19 Oct) of the US guideline: a handy exam-revision summary; compare with NICE/UK practice. No new UK SMI, Green Book, NICE infection/AMR or EUCAST changes (EUCAST’s latest news is still the 13 Oct VetCAST and 7 Oct paediatric dosing proposal items; breakpoints remain v16.1).
- CDC Level 1 Ebola travel notice for Kenya (19 Oct): relevant to pre-travel advice and assessing returning travellers (see CDC below).
- Still open: the UKAP consultation on healthcare workers living with HIV who perform exposure-prone procedures (closes 8 Nov 2026).
- Still current: MHRA DSI/2026/014 on echinocandins with CRRT (see UK section).
UKHSA / MHRA / England
Kenya and UK practice: unchanged
There is still no new UKHSA statement. The Kenya entry on the HCID country-specific risk page was last updated on 15 Oct.
Action: use it for VHF risk assessment in returning travellers, including healthcare and hospital support staff.
Week 42 still the latest
Hospital lab positivity: flu 6.0%, COVID-19 8.8%, RSV 1.0%. Week 43 is due 22 Oct 2026 at 14:00.
England: 1,030 cases
Lab-confirmed, 1 Jan–12 Oct 2026.
S. Enteritidis t5.9411: 439 cases
Data to week 41; no week 42 release yet.
Still week 41
Week ending 11 Oct; no newer release.
No new infection-relevant alert
The 19 Oct postings were routine field safety notice round-ups. DSI/2026/014 (give echinocandins later when CRRT uses new PAN-membrane filters, 13 Oct) is still current.
ECDC
DRC 9,455 / 4,561 (data to 17 Oct); all 8 Kenya cases included
- DRC: +178 cases / +83 deaths since ECDC’s 16 Oct update. Confirms Nord-Ubangi (Wapinda) as the 8th affected province.
- Summarises all 8 Kenya cases, including 1 death.
- ECDC’s modelling still puts the risk of importation into the EU/EEA at very low, with the caveat that returning healthcare workers have substantially higher exposure risk.
The Communicable Disease Threats Report for week 42 (16 Oct) is still the latest; week 43 is expected around 23 Oct. No new Eurosurveillance issue since 15 Oct (next expected Thursday 22 Oct).
Global — Ebola (Bundibugyo)
DRC: 9,552 confirmed, 4,602 deaths (data to 18 Oct, published 19 Oct)
- SitRep 156 (data to 17 Oct, published 18 Oct): 9,455 / 4,561; +40 / +25 on SitRep 155 (9,415 / 4,536).
- SitRep 157: +97 cases / +41 deaths (Nord-Kivu 48, Ituri 42, Haut-Uélé 7); CFR 48.2%. Hotspot zones: Beni 18, Vuhovi 15, Bunia 14. 30 of 41 deaths were in the community. Test positivity 19.4% (97/500).
- Recovered 2,550; 937 patients in Ebola treatment centres. 7-day average 69.6 cases/day, up from 66.0 the week before. Contact follow-up 76.9% (target 85%).
- Nord-Kivu treatment centres are over capacity (450 patients, 410 beds), and decontamination teams were assaulted in Beni.
- Ervebo vaccinations total 8,353 since 26 Aug. No new health zone since 16 Oct: still 66 zones in 8 provinces.
Check before quoting: these figures come from the ebola-tracker.org SitRep summaries and PDF, not from WHO. Ervebo is a vaccine against Zaire ebolavirus, and the digest doesn’t explain its role in this Bundibugyo outbreak.

Kenya: 8 confirmed cases, 1 death
- No ninth case reported at compile time.
- Director-General Patrick Amoth said all seven earlier patients have received monoclonal antibodies and are responding well. The eighth case (a 49-year-old patient attendant) is isolated.
- Officials still say transmission is limited to contacts of the index case. As of 17 Oct, 128 of 130 contacts of the index patient had been traced.
- There is still no approved vaccine or specific treatment for Bundibugyo virus.
Press-reported: from health ministry briefings via the media; no official ministry sitrep was retrieved.
WHO
No Disease Outbreak News on Kenya’s secondary cases
The latest DON is MERS-CoV, Saudi Arabia (15 Oct); the last Ebola DON was 8 Oct.
CDC / US
Level 1: “Ebola Bundibugyo Virus Disease in Kenya”
The notice says Kenya has confirmed an imported case plus linked cases in Nairobi. Travellers should practise usual precautions, avoid contact with symptomatic people and their body fluids, and isolate and call ahead if they develop symptoms. It links to CDC’s post-travel VHF evaluation resources. Other Ebola notices are unchanged: Level 4 Ituri/Nord-Kivu, Level 3 Haut-Uélé/Tshopo, Level 2 for the rest of the DRC and Uganda. The Ghana and Colombia yellow fever Level 2 notices (16 Oct) are unchanged.
No new health advisory
The latest is still CDCHAN-00533 (rabies PEP, 10 Sep). Checked by web search, as the archive was blocked.
Still the 15 Oct issue
No Early Release since; the next weekly issue is expected around 22 Oct.
4,410 US cases (as of 15 Oct)
Page updated 16 Oct; the next update is due Thursday.
Shamonda orthobunyavirus, Central Europe, 2026
Since mid-June, a nonfatal syndrome in dairy cattle in southern Germany and Switzerland (fever, diarrhoea, reduced milk yield) has been traced by metagenomics to Shamonda virus, a Simbu-serogroup orthobunyavirus. A veterinary One Health signal, not a human-disease report.
Journals
New since the 19 Oct issue (online 19–20 Oct). No new items in this window for NEJM infection, JAMA Network Open infection, JAC, JAC-AMR, J Hosp Infect, CMI, ARIC, Lancet Microbe, JID, AAC or JCM, and no new Eurosurveillance issue.
Lancet Infectious Diseases
Oropouche virus infection in pregnancy, Rio de Janeiro (prospective cohort)
544 pregnant women: 130 with confirmed infection, 143 possible and 271 uninfected. Adverse maternal outcomes 18% vs 7% (aRR 2.90). Fetal loss (aRR 3.08) and haemorrhage (aRR 2.79) were about three times more common; aRR for adverse infant outcomes 9.18. CNS abnormalities in 8% of infants of infected mothers vs none in the uninfected group. Symptoms did not predict adverse outcomes. An accompanying comment discusses inequality in these outcomes.
Takeaway: relevant to counselling pregnant travellers to Oropouche-affected areas and to testing in pregnancy.
Infection Control & Hospital Epidemiology
Procalcitonin in severe community-acquired pneumonia (12-hospital US cohort)
In 1,553 admissions, PCT testing within 24 h was linked to modestly fewer antibiotic days (6.9 vs 7.4; adjusted difference −0.5 days). The effect was larger in clinically stable patients (−0.8 days), and low PCT (<0.5 µg/L) was linked to 1.1 fewer days than high PCT. Testing was done regardless of clinical stability; the authors suggest reserving it for stable patients.
Open Forum Infectious Diseases
IV-to-oral switch in bloodstream infection (two multicentre cohorts)
Of 2,770 patients, 742 (19.4%) were eligible for IVOS, but only 32.3% were switched by day 7, often on the day of discharge. Diabetes, CKD, Pitt score ≥2 and septic shock were barriers; a urinary source favoured switching. IVOS was linked to a shorter stay (7 vs 12 days) and lower 28-day mortality (observational).
Takeaway: supports building IVOS prompts into AMS rounds.
742 of 2,770 is 26.8%, not 19.4%, so one of these figures may be wrong in the digest. Check the paper before quoting.
American Journal of Infection Control
Carbapenem-resistant Acinetobacter baumannii ICU outbreak (Turkey)
Recognised clinically over 6 days, but index patients had been colonised up to 41 days earlier. CRAB was found on high-touch surfaces; 6 of 7 patients died (4 CRAB-attributable). Prior A. baumannii isolation was the only independent risk factor. The epidemic clone reinfected treated patients up to 3.5 months later.
IPC lessons: extended environmental decontamination and isolation, and long-window typing.
Clinical Infectious Diseases
Extrapulmonary TB mortality by site (777,512 Indian adults)
In-treatment mortality varied more than tenfold by site. Compared with lymph-node TB, it was highest for TB meningitis (aOR 12.48), miliary TB (5.17) and pericardial TB (3.86).
Fleming Fund (UK-funded) AMR surveillance in LMICs
A series reviewing ten years of the £498 million programme across 25 countries (One Health design, data use, governance). Policy background only.
International Journal of Infectious Diseases
Successive syphilis episodes in people with HIV (Madrid prospective cohort)
362 episodes in 183 people. The 12-month serological response was similar across episodes (75–83%), but serofast status rose with repeat episodes (33% → 62–64%) and seroreversion fell. Not explained by treatment failure or neurosyphilis.
Takeaway: interpret persistent RPR titres after reinfection with care.
Zoster vaccination in immune-mediated inflammatory diseases (meta-analysis, 26 studies)
Recombinant zoster vaccine efficacy 90.5%; disease flares not significantly increased (RR 1.05). Reactogenicity was higher with RZV.
Takeaway: supports Shingrix use in IMID patients on immunosuppression (cf. Green Book shingles chapter).
On X
A minimal weekday check. No individual posts are linked.
Kenya / DRC Ebola dominated, with no new information beyond the official sources in the Agencies tab.
US avian influenza in animals: posts reported a bird flu detection in a Sacramento County (California) poultry flock, plus recirculation of the Washington State human H5N1 case (dairy exposure, reported 16 Oct; covered on 18 Oct). No new human case signal.
UK: no breaking UKHSA/NHS outbreak signal; posts were mainly flu-vaccine promotion.
Unverified: the Sacramento County poultry detection has no primary agency source yet.
What we could not confirm
We want you to know exactly where today’s issue has limits, so here are the 5 gaps we couldn’t close, plus the reports that simply weren’t out yet.
| Source | Issue |
|---|---|
| CDC | The HAN archive, newsroom and bird flu situation summary were blocked. HAN status was checked by web search (no new advisory). MMWR, EID, travel notices and measles loaded fine. |
| WHO | No Disease Outbreak News on Kenya’s secondary cases yet; WHO’s daily Ebola sitrep wasn’t checked separately. |
| Journals | NEJM’s feed returned items without titles (checked another way instead). Several IJID items were title-only (pegivirus in febrile/CNS illness; a pirfenidone COVID trial) and aren’t summarised. CMI and JHI were checked indirectly only. |
| Kenya | Figures come from health ministry briefings via the media; no official ministry sitrep was retrieved. |
| DRC | Figures come from the ebola-tracker.org SitRep summaries rather than WHO. |
