The patient is a young female complaining of increased frequency. The number of organisms in the urine was &gt;100,000 organisms\/ml.<br>What is the significance?<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>The bacteria<\/strong><\/h4>\n\n\n\n<p>&nbsp;Previously known as&nbsp;Escherichia adecarboxylata, it now belongs to a new genus Leclercia (family Enterobacteriaceae).<br>First described by Leclerc in 1962 as&nbsp;ubiquitous in nature (Food, water etc., animals \u2013 snails).<br>It is also a member of the gut flora (commensal).<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Identification<\/h4>\n\n\n\n<p>A motile, aerobic, Gram-negative bacillus<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table><tbody><tr><td><strong>Positive for<\/strong><\/td><td><strong>Negative for<\/strong><\/td><\/tr><tr><td><em>Indole production,<\/em>&nbsp;<em>methyl red,<\/em>&nbsp;<em>growth in the presence of KCN,<\/em>&nbsp;<em>malonate utilization,<\/em>&nbsp;<em>esculin hydrolysis,<\/em>&nbsp;<em>gas and acid production<br>from D-glucose and D-lactose<\/em><\/td><td><em>Voges-Proskauer,<\/em>&nbsp;<em>citrate (Simmons),<\/em>&nbsp;<em>H2S (Kligler),<\/em>&nbsp;<em>amino acid decarboxylases,<\/em>&nbsp;<em>phenylalanine deaminase,<\/em>&nbsp;<em>gelatinase and<\/em>&nbsp;<em>DNase<\/em><\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p>Modern identification methods like&nbsp;MALDI-TOF&nbsp; and 16 s rRNA gene sequencing&nbsp;can distinguish it from phenotypically similar organisms.<\/p>\n\n\n\n<figure class=\"wp-block-image\"><img decoding=\"async\" src=\"https:\/\/microregistrar.com\/wp-content\/uploads\/2021\/07\/image-8.jpeg\" alt=\"Leclercia adecarboxylata colony\" class=\"wp-image-6490\"\/><figcaption class=\"wp-element-caption\"><a href=\"https:\/\/microregistrar.com\/wp-content\/uploads\/photo-gallery\/Picture1.jpg?bwg=1604336391\"><strong>Leclercia adecarboxylata colony<\/strong><\/a><\/figcaption><\/figure>\n\n\n\n<figure class=\"wp-block-image\"><img decoding=\"async\" src=\"https:\/\/microregistrar.com\/wp-content\/uploads\/2021\/07\/image-9.jpeg\" alt=\"Leclercia adecarboxylata gram stain\" class=\"wp-image-6491\"\/><figcaption class=\"wp-element-caption\"><a href=\"https:\/\/microregistrar.com\/wp-content\/uploads\/photo-gallery\/Picture3.jpg?bwg=1604336391\"><strong>Leclercia adecarboxylata gram stain<\/strong><\/a><\/figcaption><\/figure>\n\n\n\n<h4 class=\"wp-block-heading\">Pathogenesis &amp; pathology<\/h4>\n\n\n\n<p>Leclercia is an environmental organism but also a member of human bowel flora.<br>It is considered an opportunistic pathogen, and most cases are reported from immunocompromised patients [<a href=\"https:\/\/www.microbiologyresearch.org\/docserver\/fulltext\/jmm\/57\/7\/896.pdf?expires=1604325707&amp;id=id&amp;accname=guest&amp;checksum=4A37A88482811C02985AB2514C46E47F\">Hess, 2008<\/a>].<br>It is often associated with polymicrobial infection, especially in immunocompetent hosts.<br>It is a possibility that the organism is often misidentified as E coli as they share many biochemical features.<\/p>\n\n\n\n<p><strong>The reported cases are \u2013<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Pharyngeal and peri-tonsillar abscess [<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/23324820\/\">Bali et al., 2013<\/a>],&nbsp;<\/li>\n\n\n\n<li>Endocarditis with embolic complication [<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/19214035\/\">Bora el al, 2009<\/a>]<\/li>\n\n\n\n<li>Polymicrobial wound infection and pneumonia [<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/9243038\/\">Temesgen et al., 1997<\/a>] [Greco et al., 2001]<\/li>\n\n\n\n<li>Cholecystitis (polymicrobial) [<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/11283116\/\">de Baere 2001<\/a>]<\/li>\n\n\n\n<li>UTI With Ent faecalis in case of bladder tumour [Sawamura et al., 2005]<\/li>\n\n\n\n<li>Bacterial peritonitis in a peritoneal dialysis patient [<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/11446916\/\">Rodriguez et al. 2001<\/a>]<\/li>\n\n\n\n<li>Bacteraemia in the presence of<br>\u2013 preexisting diseases like&nbsp;cancer, leukaemia, renal failure, haematopoietic stem cell transplant and cirrhosis or<br>\u2013&nbsp;conditions associated with skin barrier dysfunction, including&nbsp;trauma, burn wounds, peritoneal dialysis and cellulitis [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4316257\/\">Anuradha 2014<\/a>] [Matsuura 2018]&nbsp;[<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/9243038\/\">Temesgen et al., 1997<\/a>]<\/li>\n<\/ul>\n\n\n\n<p>In some cases, the bacteremia associated with Leclercia has been reported to be polymicrobial.<\/p>\n\n\n\n<p><strong>So, in summary, Leclercia can cause various infections<\/strong>, usually associated with an immunocompromised state; however, infections have<strong>&nbsp;been reported in immunocompetent people. Infections are often polymicrobial.<\/strong><\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Treatment<\/h4>\n\n\n\n<p>Leclercia adecarboxylata is susceptible to most common antibiotics; however, resistance to various antibiotic classes has been reported.<br><a href=\"https:\/\/www.clinicalmicrobiologyandinfection.com\/article\/S1198-743X(14)62769-5\/fulltext\">Stock&nbsp;<\/a>et al.&nbsp;analysed the natural susceptibility of 101 strains by broth microdilution. The pattern is not unexpected from Enterobacteriaceae except for natural resistance to fosfomycin.<br>Note \u2013 the only azithromycin amongst all macrolides has been listed as susceptible. Dicationic azithromycin penetrates Enterobacteriaceae hydrophilic outer membrane and acidic lipopolysaccharide more efficiently than other hydrophobic macrolides.<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table><tbody><tr><td><strong>Naturally sensitive to<\/strong><\/td><td><strong>Naturally resistant to<\/strong><\/td><\/tr><tr><td>Tetracyclines,<br>Aminoglycosides*,<br>All but two beta-lactams,<br>Quinolones*,<br>Trimethoprim, Cotrimoxazole, Chloramphenicol,<br>Nitrofurantoin and Azithromycin.<\/td><td>Penicillin G, oxacillin,<br>Erythromycin, roxithromycin, clarithromycin, ketolides,<br>Lincosamides,<br>Streptogramins,<br>Linezolid,<br>Glycopeptides,<br>Rifampicin, fusidic acid and fosfomycin.<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p>ESBL production and resistance to aminoglycosides, quinolones, and aztreonam have been reported by various authors&nbsp;[<a href=\"https:\/\/www.researchgate.net\/figure\/Case-reports-of-pediatric-patients-with-Leclercia-adecarboxylata-infections_tbl1_281520787\">Hurley 2015<\/a>]<\/p>\n\n\n\n<p>Cristina Riazzo (2017) reported a case of NDM from Spain, which was resistant to&nbsp;all beta-lactams, fluoroquinolones, trimethoprim-sulfamethoxazole, gentamicin and tobramycin, and only remained sensitive to tigecycline, amikacin and colistin.&nbsp;This patient was successfully treated with Imipenem\/ cilastatin and amikacin.
