It prevents the formation of new cell walls and maintenance of the cell wall leading to autolysis of the bacteria due to osmotic pressure.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-mechanism-of-resistance\">Mechanism of resistance<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Beta-lactamase: Secreted beta-lactamase of the gram-positive bacteria or the beta-lactamase in the periplasmic place of the gram-negative bacteria can hydrolyse cefalexin.<\/li>\n\n\n\n<li>Altered PBP: alteration of PBP may lead to cefalexin resistance, e.g. MRSA.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-pk-pd-and-other-properties\">PK\/PD and other properties<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Bioavailability:<\/strong>\u00a0 High. 82% at the fasting stage and 73% with food.<\/li>\n\n\n\n<li>Cefalexin absorption is not impaired by heart failure, achlorhydria, or partial gastrectomy.<\/li>\n\n\n\n<li><strong>Protein binding:<\/strong>&nbsp;15%<\/li>\n\n\n\n<li><strong>Distribution:<\/strong>\u00a0High in the kidney, Good concentration is inflamed lung, also found in the pancreas, liver and adrenals.<\/li>\n\n\n\n<li><strong>Important parameter:<\/strong>&nbsp;Time over MIC.<\/li>\n\n\n\n<li>Metabolism: Not metabolised in the body.<\/li>\n\n\n\n<li>Excretion: Mostly kidney, &lt;1% in bile.<\/li>\n\n\n\n<li>Dose adjustment:&nbsp;Necessary in renal impairment<\/li>\n\n\n\n<li>Pregnancy: Category B<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-indication-cefalexin-is-use-to-treat\">Indication (cefalexin is used to treat)<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>&nbsp;Urinary tract infections,<br>&nbsp; \u2013 including acute pyelonephritis (not as initial therapy but as oral stepdown)<br>&nbsp; \u2013 catheter-associated UTI<br>&nbsp; \u2013 uncomplicated prostatitis<\/li>\n\n\n\n<li>Prophylaxis of recurrent urinary tract infection (unlicensed use)<br>&nbsp; &nbsp;\u2013 (not preferred for VUR prophylaxis, Ref \u2013 Kucer)<\/li>\n\n\n\n<li>Diverticular disease (with metronidazole)<\/li>\n\n\n\n<li>Respiratory tract infections (directed therapy)<\/li>\n\n\n\n<li>Skin and soft tissue infections (not animal bites)<\/li>\n\n\n\n<li>Otitis media (directed therapy)<\/li>\n\n\n\n<li>OM and septic arthritis (sequential therapy)<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-spectrum\">Spectrum<\/h2>\n\n\n\n<p><strong>Gram-positive bacteria:<\/strong><\/p>\n\n\n\n<figure class=\"wp-block-table\"><table><tbody><tr><td><strong>Susceptible<\/strong><\/td><td>MSSA, Group A Streptococcus, Group B Streptococcus, Viridans Streptococcus.
