This mechanism is unique; hence, no cross-resistance with other antibiotics has been reported.<br>It prevents bacterial protein synthesis.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Mechanism of resistance<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Low-level resistance (MIC 8-256) \u2013 Low-level resistance is chromosomal; it is due to\u00a0point mutation in\u00a0ileS\u00a0gene. This gene codes for isoleucyl tRNA synthetase.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>High-level resistance (MIC>512) \u2013 Plasmid; \u00a0mupA gene mediated \u2013 which generates additional isoleucyl tRNA\u00a0transferase.<\/li>\n\n\n\n<li>The risk of resistance is higher in cases of \u2013 long-term use, use in the presence of a foreign body (NG tube), and decubitus ulcer.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>&nbsp;PK\/PD<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Inactivated in plasma, and hence it is only used topically.<\/li>\n\n\n\n<li>Bacteriostatic\u00a0near MIC and bactericidal at higher concentrations.<\/li>\n\n\n\n<li>Poorly absorbed when used topically, absorbed drugs get\u00a0metabolised\u00a0in the liver.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Spectrum<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Staph aureus (including MRSA, VISA, VRSA), strep, CoNS,\u00a0Enterococci, and Streptococcus\u00a0bovis\u00a0have high MIC,<\/li>\n\n\n\n<li>Micrococcus resistant.&nbsp;Corynebacterium and anaerobic gram positives are resistant.<\/li>\n\n\n\n<li>Haemophilus, Moraxella, Neisseria, Bordetella, Pasteurella sensitive.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Adverse effect<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Burning, stinging, itching, rash, headache.<\/li>\n\n\n\n<li>Mupirocin&nbsp;prep containing polyethylene glycol should be used with caution in renal impairment.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Use<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Eradication regimen for MRSA \u2013 anterior&nbsp;nares&nbsp;BD X 5 days.
